The expression "Closing the barn door after the horse got out" applies to many safety programs. Many safety engineers are so busy responding to safety incidents they don't have the time to spend on preventing accidents from happening. A safety incident can be a catastrophic event, or a seemingly insignificant event, or near-miss. How many near misses occur before one catastrophic event occurs?
